Use this form to report your UFO or Alien Sighting. If you are reporting an abduction, please use the Alien Abduction Form.

Note: Due to the sensitive nature of this subject, Mystical Universe will not accept submissions unless accompanied by an accurate email address, and we will NOT publish any submissions containing offensive language.

Please try to answer all questions, but do not try to fill in gaps that you cannot recall.
Accuracy is the key, not quantity.

Before starting, we suggest that you look through the form to familiarize yourself with it. We also suggest that you write out your detailed sighting/experience in notepad, Microsoft Word, or your favorite text editor, and then copy and paste the information into the form. This way you have a copy of it, in case something goes wrong when you send the form, or if you accidently reset the form, lose internet connection or accidently close the window.

Body - overall shape, height, weight, color, hairy, scaly ...
Appendages - what did they look like" Arms were long, short, no arms ...
Shape of head, eyes, mouth....
Did it have an odor, did it make sounds - did it sound like it was trying to communicate...
Did it seem hostel, calm, curious, scared....
